Summary:
As the Director of Case Management, you'll oversee department activities, ensure compliance, and coordinate patient care processes for optimal outcomes.
Responsibilities:
Direct and evaluate departmental operations to achieve performance objectives.
Establish working relationships with key stakeholders for effective care coordination.
Assess department performance and implement improvement activities.
Ensure compliance with policies, SOPs, and care coordination processes.
Qualifications:
Bachelor's degree required; Master's degree preferred.
RN licensure or LCSW required.
5-7 years' experience in acute care Hospital Case Management.

years' experience in case management leadership preferred.
