We are currently seeking a dynamic and detail oriented individual for the position of Patient Care Coordinator in our Donor/Third Party IVF department.
Duties Include but not limited to:
Case Management of third party IVF cycles Coordinating patient care plans by liaising with all necessary care providers to ensure patient needs are met o Contact potential donors for pre screening o Collect and maintain medical records for cycle clearance o Make sure consent forms and other cycle related documents are completed and archived o Create and maintain egg donor profiles o Serve as point of contact for Egg Donors, Recipients, and all outside parties involved o Monitor patient's progress during IVF cycle and follow up on any missing items Work to create efficient systems and workflows within the department Additional tasks/projects related to the role Skills and
Qualifications:
Minimum of 2 years working in a clinical or social work setting Excellent computer skills Familiarity with electronic medical records software preferred Strong organizational and time management skills High Level of customer service Attention to detail and independent Education:
Minimum of High School degree required.
College degree strongly preferred.
