Summary:
The Care Manager RN will interact with the multidisciplinary health care team to ensure care coordination occurs for the primary care patient population across the care continuum by (1) ensuring the delivery of needed care services and the removal of barriers to care, (2) assisting patients in overcoming obstacles across the patient's care continuum, and (3 building therapeutic and trusting relationships with patients, families, and all caregivers.
This position provides clinical functions as required for this patient population and participates in optimizing patient care management and providing a patient care experience that is safe, timely, efficient, cost-effective, equitable and patient-centered.
Within the established guidelines set forth by Agape Family Health and under the direction of the appropriate administrator or manager, ensures adherence to all policies, procedures, and regulatory requirements.
Essential Duties and
Responsibilities:
Promotes a patient- and family-centered care environment for ethical decision making.
Advocates for patients to promote optimal care and outcomes.
Promotes autonomous decision making by patients.
Disease management for patients with chronic health conditions Plans and implements age-appropriate care based on assessment, diagnosis, standard of care, medical treatment plan, and progress toward desired outcome.
Assesses individual physical, psychosocial and developmental needs of patients.
Establish positive and constructive business relationships with health plans, external pharmacies, specialists, and other interdisciplinary care teams.
Works with designated physicians and other healthcare professionals to develop and maintain clinical protocols, to include coordinating their entry into the electronic medical record (EMR).
Assumes responsibility and accountability for tracking lab, pathology, cytology and radiology procedures ordered to ensure results are received and viewed by the responsible provider.
Maintains open communications with all health team members (both internal and external) on behalf of the patient, and their significant others as designated by the patient.
Notifies providers to confirm patient exams ordered/required, obtains prior exams/films/results, and ensures tests, procedures and related consultations are scheduled and performed.
Assesses educational barriers and needs of patients, families, caregivers, and provides education that best supports the understanding of the diagnosis and plan of care.
Provides care education to healthcare team members.
Works closely with physicians and allied health professionals in all areas (both internal and external) to coordinate, communicate and update, and facilitate all components of the patient's multidisciplinary plan of care.
Serves as the patient's central point of contact.
Triage calls for clinical providers (collects and assesses patient data to determine patient needs and to promote optimal patient care); documents calls in the electronic medical record.
Calls in prescriptions to pharmacies as directed by appropriate members of the care team.
Participates in the tracking of metrics and patient outcomes, in collaboration with administration.
Participate in the training /education of new health center staff in care coordination strategies.
Assist with the Care Management initiatives within the NCQA PCMH accreditation.
Provide direct support onsite or telephonically to providers, Health Center Office Managers, and staff to ensure optimal care coordination for AFH patients.
Train and motivate staff to effectively address quality-related gaps in care.
Identify barriers to productivity and quality; appropriately escalate concerns to management in a timely manner.
Comply with HIPAA requirements for patient confidentiality.
Provide customer service of high quality maintaining effective, timely and respectful communication with patients, other employees, Medical Assistants and providers.
Effectively promote the benefits of compliance with preventive care, as well as disease management through the education and monitoring of patients.
Have flexible working hours to contact patients after working hours if needed Comply with all health and safety policies of the medical practice, Other duties as needed and/or assigned.

Education and Experience Requirements:
Current State or Compact Licensure as RN BSN preferred 1-2 years experience in care coordination, CCM preferred High School Diploma / GED Current CPR certificate Excellent interpersonal, communication and time management skills Ability to work in fast paced environment with multiple health care providers
