7 Quick Apply Full-time 1 day ago Full Job Description The Behavioral Health Care Coordinator is a member of the Behavioral Health Care Management team whose role is to perform telephonic outreach and screenings of identified patients and to provide coordination of Behavioral Health services and community resources.
The Behavioral Health Care Coordinator supports patients and providers across service areas, identifies facilities and programs, and advocates and coordinates care for the patients to ensure access to Behavioral Health services and to community resources that address social determinants of health.
The Behavioral Health Care Coordinator triages for and collaborates with clinical staff to ensure patients receive appropriate care management.
The Behavioral Health Care Coordinator collaborates with the patient's clinical team and other internal departments to support Patient needs.
Key Functions Uses motivational interviewing skills to engage patients into care management via telephonic and/or face to face outreach Collaborate with medical, behavioral, and social support staff to enhance usual primary care particularly to patients whose conditions aren't improving Behavioral health care planning concerning behavioral or psychiatric health problems Revisions of patient's care plans that are not progressing or whose status changes Collaborative, integrated relationship with the rest of the care team Assesses treatment adherence, tolerability, and clinical response using validated rating scales Delivers brief evidence-based psychosocial interventions such as behavioral activation or motivational interviewing.
Provide at least 70 minutes of behavioral health care coordinator time during the first month of services Provide at least 60 minutes of behavioral health care coordinator time during all following months.
Conduct screening and risk assessment interventions per program guidelines Provides information to patients with the goal of increasing Patient knowledge and participation in their own healthcare management including but not limited to information on how to obtain resources; basic health information; information packets containing health information relative to the Patient's identified condition Maintains current knowledge of community resources for referral and linkage to meet Patients' needs Advocates for the Patient and coordinates BH and community resources Facilitates referrals and provides appointment, as needed Coordinates and facilitates access to BH services, resolves issues related to social determinants of health and collaborates with providers, facilities, state agencies, BH CCM team, and other departments Maintains accurate and timely documentation in the medical management information system in keeping with contractual requirements, internal policy and accreditation standards Responsible for preparing department data/reports assigned by Management Identifies opportunities for improvement in administrative workflows and processes Performs other associated tasks as assigned by Manager/Supervisor Quick Apply.
